In partnership with Ng Teng Fong General Hospital, ADA invites you to a free community health forum on 21 and 22 September to learn more about the early detection and treatment of Alzheimer’s Disease.

 

Alzheimer’s Disease is the most common form of dementia in Singapore. Today, dementia affects more than 45,000 Singaporeans. A person with dementia may have problems with memory, thinking skills, emotions, behaviour and mood. As a result, a person with Alzheimer’s gradually loses their ability to carry out daily activities. An early diagnosis means you can live well for longer.

Come and listen to our lineup of experts in the field, and gain a better understanding about dementia. Apart from the public talks, visitors can attend workshops, enjoy performances and take part in physical exercises. There will be booths by various partners, including Fei Yue, Forget Us Not, Jaga-Me, SAGE Counselling Services and the Singapore Association for Mental Health.

Dementia is a disease that affects the brain and its ability to function. Through this talk, Dr Koh will share an overview of Dementia to better understand the condition.

The Patient’s Journey

Ms Janet Bay, Case Manager, Ng Teng Fong General Hospital
(Presented in English and Mandarin)

A Patient’s Journey details the sequential check-points that a person goes through across the care continuum. Case manager Janet will share with us a real-life case study of how a person with dementia is being cared for, with the support of community care partners.  Janet will be delivering her talk in English, followed by Mandarin.

Dr Tan Yuyuan, Associate Consultant, Psychiatry, Ng Teng Fong General Hospital

This talk provides an overview of common behavioural expressions and psychological symptoms observed in persons with dementia, including an understanding of the possible underlying causes, complications and treatment available in managing these symptoms.

Dr Kenny Cheong, Psychologist, Ng Teng Fong General Hospital

This talk covers the bi-directional effect of BPSD on patients and caregivers with insights from a person-centred care perspective and the various non-pharmacological strategies that caregivers can adopt to care for family members with BPSD.

Forget Us Not – a Dementia-friendly Community

Mr Ivan Loh

Dementia is a disease that affects the lives of patients and their families in many aspects. In this talk, brought to you by “Forget Us Not”, an independent initiative organised by Khoo Teck Puat Hospital, ADA and Lien Foundation, learn about this campaign which aims to raise awareness of the increasing number of people with dementia and their daily struggles, and the importance of building a Dementia Friendly Community (DFC) to support them.

Mobilising Persons with Dementia

Mr Low Weng Hoe, Physiotherapist, Jurong Community Hospital

Dementia is a chronic progressive disease. As the disease progresses, persons with dementia often have decreased physical mobility. Promoting physical mobility in people with dementia not only helps in improving physical functions, it also retains independence and creates a sense of purpose and well-being.

Home Modification for Persons with Dementia

Mr Too San Muan, Occupational Therapist, Jurong Community Hospital

Persons with dementia often experience symptoms such as forgetfulness and changes in visions such as depth perception and contrast sensitivity. These could lead to frequent falls and agitation of the elderly. Occupational therapist Mr Too San Muan will share how simple modifications to the home environment can help to provide a safe premise for persons with dementia and encourage them to maintain their independence at home.

Caring for persons with dementia can be an overwhelming experience. It is important for caregivers to know the what, when, who and how of utilising resources available in the community to aid them in caring for persons with dementia.

Senior Consultant, Family Physician & Director of Service, Post-acute and Continuing Care, Jurong Community Hospital

Dr Kelvin Koh currently practices as a Senior Consultant Family Physician and Director of Service, Post-acute & Continuing Care in Jurong Community Hospital. His experience in the Intermediate-Long-term Care sector started in 2005 and he has developed a keen interest in dementia care whilst serving in this sector. He contributes actively to the dementia care team in Jurong Community Hospital and is a member of Jurong Health Campus’s Dementia Programme project team.

Dr Koh holds several teaching appointments and is involved in undergraduate and postgraduate teaching in Family Medicine and Geriatric Medicine. Dr Koh also serves as a continuing medical education advisor for family medicine. In 2015, he was presented with the Distinguished Educator’s Award from the College of Family Medicine Singapore in 2015 for his contribution to family medicine teaching.

Case Manager, Ng Teng Fong General Hospital

Having worked in the healthcare sector for the past 17 years, SSN Janet Bay developed a keen interest in working with the elderly and subsequently went to obtain an advanced diploma in Gerontology. As a case manager supporting dementia care, Janet serves as an advocate for patients and helps to coordinate care services for the patient to aid them in transiting back to the community seamlessly. Janet also conducts caregiver training and psychoeducation for caregivers of dementia patients.

Associate Consultant, Psychiatry, Ng Teng Fong General Hospital

Dr Tan Yuyuan is an Associate Consultant in the Department of Psychiatry, Ng Teng Fong General Hospital. She obtained her MBBS from the National University of Singapore Yong Loo Lin School of Medicine, and completed her postgraduate Masters of Medicine (Psychiatry) in 2014. She has a special interest in old age mental health and is a member of the hospital’s dementia workgroup.

Psychologist, Ng Teng Fong General Hospital

Dr Kenny Cheong is a psychologist in the Department of Psychology Services at Ng Teng Fong General Hospital. He obtained his Doctor of Psychology (Clinical and Clinical Geropsychology) from James Cook University, Townsville and has a special interest in working with older adults. Dr Cheong is also a fully registered psychologist under the Psychology Board of Australia.

Campaign Administrator, Forget Us Not Campaign

Mr Ivan Loh was trained in eldercare and has a special interest in dementia, due to personal experiences within the extended family. He was attached to Khoo Teck Puat Hospital (Geriatrics Clinic) for 8 months, working with a team of dedicated healthcare professionals serving dementia patients. As part of his current work, he gives talks to different organisations to raise awareness about dementia.

Physiotherapist, Jurong Community Hospital

Mr Low Weng Hoe completed his Bachelor’s degree in Physiotherapy from Trinity College of Dublin. He currently works in Jurong Community Hospital as a Physiotherapist, and has a special interest in physiotherapy for the geriatric population and persons with dementia. Mr Low was awarded the Social and Healthcare Manpower Development Programme (HMDP) award for Dementia Care in 2016.

Occupational Therapist, Jurong Community Hospital

A senior occupational therapist with more than 10 years of experience, Mr Too San Muan holds a Bachelor Degree of Health Science (Occupational Therapy) from St Loyes School of Health Studies, affiliated to Exeter University. He also conducts caregiver training and was involved in the set up of the Dementia Caregiver Support Programme. Mr Too was awarded the Social and Healthcare Manpower Development Program (HMDP) Award in Dementia Care, in 2016

Principal Medical Social Worker, Ng Teng Fong General Hospital

Ms Kong Lai Yee is a registered social worker with decades of experience working with families of persons with dementia and geriatric patients. Ms Kong also supports the Assessment and Shared Care Team (ASCAT) in NTFGH, which is a multi-disciplinary team led by a psychiatrist to bridge psychiatric care between the community and the acute hospital.
