Post Diagnostic Support Plus (PDS+)

Post Diagnostic Support Plus (PDS+) is a pilot programme that aims to provide early precision person-centred palliative care for persons with dementia and their caregivers.

The PDS+ programme is supported by a palliative care team that provides transitional care support for persons with dementia who are no longer suitable for dementia day care due to advancing symptoms and changing care needs. The team also provides support and consults for cases with complex care situation and needs within Dementia Singapore.

Early care planning conversations are initiated to provide proactive support and care management, rather than limiting palliative care to the advanced stages of dementia or end of life.

Transitional care support from dementia day care centre to home will include:

  • Advance care planning conversations
  • Management of behaviours of concern
  • Prescription of meaningful activities
  • Supporting the care challenges faced in the advanced stage of dementia
  • Linking families up with the relevant resources/services

Beyond direct support for persons living with dementia and caregivers, PDS+ programme seeks to strengthen sector capabilities in providing palliative care for persons with dementia. This includes developing a palliative care toolkit to share the knowledge and resources with the dementia care sector. The toolkit will be made accessible to the public progressively from 2027.

PDS+ programme is funded by Lien Foundation and supported by Khoo Teck Puat Hospital.
